My HD floppy drive from Commodore uses a small push-button type sensor to
determine if the hole is there or not. My Power Computing XL drive also
uses a similar mechanism.
But along similar lines as the dirty LED, perhaps the button is stuck in
the pushed-in position which would always indicate a low-density floppy.
